This form is a request to film or take photographs at UCLH. It does not constitute agreement nor does it imply permission to film/photograph. A separate agreement may be drawn up if permission is granted for this project to go ahead. Please note we do not normally provide parking facilities or props.

Give a short brief of your project. Generally, we like to have an idea of the main premise of your project and how access to UCLH will contribute to your project. Required

Have you discussed your project with anyone at UCLH? If yes, please list names and contact details. Required

Explain what images you would like to capture and what access you will need to do so. Required

If you have a specific location in mind please specify, otherwise give us a rough idea of what sort of location you are hoping to use, giving as much detail as possible.

Specify a date and how long your shoot will take. Required

Do you want to interview staff? If yes, who? Required

Do you want to interview patients? If yes, how many and what will you ask them? Required

List the names of your crew, and identify the person in charge on the day. Required

Please also give a mobile contact number just in case we need to reach you on the day of the shoot. Not more than two people will be allowed in clinical areas. Crew should carry ID while at UCLH.

Production company, name and address Required

Has the project been commissioned? If yes, please include name and address of commissioning organisation. Required

Equipment and/or props list Ideally your kit should not include lights or require access to the building power. For big shoots, you will be asked to provide a detailed risk assessment. Required

How will the images be used? Please include link to website and expected broadcast or publication date, as well as target audience. Required
