Every person with cancer should have access to Personalised Care and Support Planning. It is recognised that receiving care that is tailored to your individual needs can significantly impact your quality of life and your experiences during your cancer pathway.
Personalised Care and Support Planning involves 4 interventions. These will be offered at the appropriate point(s) of your care and may differ between patients.
- Holistic Needs Assessment (HNA) and Personalised Care Plan

- Health and Wellbeing Information and Support
- Cancer Care Review
The Holistic Needs Assessment (HNA) is a simple questionnaire, specially designed to support people living with cancer. The assessment is completed electronically (those who do not wish to complete it electronically – please discuss with a member of staff alternative ways of completing) The HNA helps to identify your greatest needs and concerns and ensures your physical, practical, emotional, spiritual and social needs are discussed and addressed in a timely and appropriate way. Your answers from the HNA questionnaire will then be used to develop a care and support plan for you, together with one of the health care professionals involved with your care.
A HNA will be completed at the key point(s) in your treatment pathway that will be most beneficial for you as an individual.

A Treatment Summary is a document completed by a member of the team after the end of an acute phase of your treatment (not necessarily at the end of all your treatments).
Your Treatment Summary will include details of the surgery you have had, potential outcomes/consequences of treatment, and signs and symptoms of recurrence. It will be shared with you and your GP and helps to improve communication between the hospital and your GP.
The Treatment Summary provides your GP with up to date information and will inform them of any actions that need to be taken by them. It also helps them to better understand your treatment when they meet with you to do a Cancer Care Review.

Health and Wellbeing Information and Support Events are designed to help you and your family and friends get the support you need after cancer treatment. They give the holistic information needed to enable rehabilitation and self-management.
Health and Wellbeing Information and Support Events focus on promoting healthy lifestyles, and support you to live as active and full a life as you wish and are able.
Our Health and Wellbeing Information and Support Events cover a variety of topics and provide an opportunity for you to be with others in similar situations.

Your GP should offer you a Cancer Care Review within the first 6 months of you receiving your cancer diagnosis. This Cancer Care Review provides you and your GP to discuss your cancer experience, treatments and support you to self-manage.
